Opening your fillable form on mobile
Follow the steps below for your device to fill out this form properly.
iPhone (iOS)
Using a home screen shortcut? Before adding the website to your home screen, tap the share icon in Safari, select Add to Home Screen, and make sure Open as Web App is deselected. If this option is enabled, fillable PDF fields may not be editable.

Tap the download link — when prompted, tap Save to Files and choose a location such as On My iPhone or iCloud Drive.

Open the Files app and navigate to where you saved the file. Tap it to open.

At the bottom of the screen, tap the
icon at the bottom of the screen. This opens the editing toolbar.

Tap any field in the form to begin filling it out. Tap Done when finished — your changes are saved automatically.
Android

Tap the download link — when prompted, tap Download to save the file to your device.

Open your Files app (or My Files) and navigate to Downloads.

Tap the PDF file. If prompted to choose an app, select Adobe Acrobat.

The form will open in Adobe Acrobat — tap any field to begin filling it out.
Don't have Adobe Acrobat? Download it for free from the Google Play Store. Opening the PDF in your browser's built-in viewer may not support fillable fields.
